Predominantly dependent: The person needs help planning the routine daily schedule but is able to signal agreement or rejection to structuring suggestions. They frequently cannot keep to their own plans as these are forgotten. Reminders or prompting throughout the day are therefore required. A person who can plan and decide independently but is so severely impaired that they need personal assistance for any implementation of self-planned activities is also rated as predominantly dependent.
Dependent: Participation in planning the daily structure or orientation to given structures is not or only minimally possible.

Maintaining a day–night rhythm according to individual habits and ensuring adequate rest and sleep. This includes the ability to recognise the need for rest, to rest and to cope with periods of sleeplessness, as well as somatic functions needed to get into bed. Nocturnal disease management measures are not assessed in this criterion.

Independent: The person can perform the described activity without personal assistance.
Predominantly independent: The person needs personal help getting up or going to bed, e.g. transfer help or temporal orientation help when waking, or prompting to go to sleep. Or a need for help arises at night as a rule weekly but not daily.
Predominantly dependent: Sleep difficulties or nocturnal restlessness occur regularly (almost every night) and the person is largely unable to cope alone. Elaborate bedtime routines or calming speech at night are therefore required. A person who needs personal assistance almost every night to be able to continue sleeping, e.g. for position changes or toilet trips at night, is also predominantly dependent.
Dependent: The person has no or a disturbed sleep–wake rhythm. This applies to, among others, mobile persons with gerontopsychiatric conditions and people who perform no activities at all, e.g. in a vegetative state, or persons who regularly need personal support at least three times a night.

Keeping oneself occupied

Accepted Answer

Using available time for activities according to one's own preferences, e.g. handicrafts, reading or entertainment electronics. The ability to select and carry out suitable activities is assessed.

Independent: The person can perform the described activity without personal assistance.
Predominantly independent: Only minor assistance is required, e.g. preparing, directly laying out and arranging objects such as a daily newspaper, headphones or similar items, reminders about customary activities, motivation or support in decision-making (making suggestions).
Predominantly dependent: The person can participate in activities but only with comprehensive guidance, accompaniment or motor support.
Dependent: The person cannot contribute meaningfully to deciding or performing activities. They show no initiative, cannot cognitively implement instructions and prompts, and do not or only minimally participate in offered activities.

Making future-oriented plans

Planning beyond the day, e.g. having wishes about upcoming celebrations or being able to estimate time sequences.

Predominantly independent: The person makes plans but needs to be reminded to carry them out. Predominantly dependent: The person does not plan spontaneously but decides with support from others. They need to be reminded to implement their own decisions or need emotional or physical support for implementation. A person who can plan and decide independently but is so severely impaired that they need personal assistance for any implementation of self-planned activities is also rated as predominantly dependent.
Dependent: The person has no concept of time for planning beyond the day; even when given options to choose from, neither agreement nor rejection is signalled.

Predominantly dependent: The person rarely takes initiative. They need to be spoken to or extensively motivated, but then respond verbally or clearly recognisably through other forms of communication (eye contact, facial expression, gestures). A person who requires comprehensive support to overcome speech, language or hearing difficulties is also predominantly dependent.
Dependent: The person does not respond when spoken to. Non-verbal contact attempts, e.g. touch, also produce no significant reaction.
